About Us

Sac Family Connect is a collaborative network whose vision is to improve the physical and emotional well-being of prenatal and parenting families through a coordinated system of care, focusing on high-quality family support services that are equitable, easily accessible, culturally responsive, and best meets the needs of those being served.

Sac Family Connect meets quarterly and is comprised of a network of more than 30 agencies across Sacramento County that offer home visiting and other family support services, with First 5 Sacramento serving as the backbone agency of the collaborative.
